Care Manager (Full-Time) Quality/Care ManagementWest Des Moines,Iowa Apply Description Collaborates with all members of the multi-disciplinary healthcare team to ensure the delivery of high quality, cost effective care, as evidenced by coordinated transitions of care; achievement of select quality outcome metrics; appropriate utilization of health management programs and resources, within the structure of a patient-centered medical home. Strong clinical background required, RN or CMA experience required. Two years clinical experience required, specialty-specific experience preferred. Experience in a case management role strongly preferred. Essential Functions + High Risk Patient Management: + Collaborate with the provider, healthcare team, and patient/family to evaluate current health status, develop a patient specific care plan to reduce barriers, promote education and self care, secure needed health care and community resources, and avoid inpatient admissions and emergency department visits. Ensure care plan consistency across providers. + Communicate with patients regularly to counsel on disease related symptom management, address compliance with care plans, and advise patients on lifestyle choices to improve prognosis. + Proactively connect patients to Payor and community health management resources and programs, as patient needs require. + Verify chronic disease conditions are listed accurately in EMR, enter chronic disease conditions accurately in EMR. + Transitions of Care: + Provide follow up on inpatient admissions and emergency department vists; working collaboratively with the provider and healthcare team to establish plan of care post discharge, including follow up appointment in the clinic to prevent unnecessary re-admissions/visits. + Tracks quality outcome results for defined population, and shares the results with stakeholders, including the provider, healthcare team, and TIC leadership. Completes chart audits on all patients in the defined population, identifying opportunities for care. Shares the results of the chart audits and care opportunities with the provider, healthcare team, clinic leadership. + Collaborates with providers, clinic staff and clinic leadership to identify needs and resolve day to day issues. Serves as a liaison between payors and the provider. + Maintains awareness of own safety in carrying out duties and responsibilities. Adheres to universal precautions and infection control policies and procedures. + Assumes accountability for enhancing own knowledge through professional associations, journals, workshops, inservices, etc. Keeps professional license current. + Maintains patient confidentiality and uses discretion in patient interactions.
